Job summary

A healthcare provider in Arizona is seeking an Occupational Therapist to deliver person-centered therapy and support services to individuals with developmental disabilities. You will conduct evaluations, develop treatment plans, and work collaboratively with interdisciplinary teams. Responsibilities

  • Conduct DDD-compliant occupational therapy evaluations.
  • Develop and implement Individualized Treatment Plans (ITPs).
  • Provide direct OT services in various settings.
  • Recommend and train on assistive technology, adaptive devices, and environmental modifications.
  • Participate in Individual Support Plan (ISP) meetings and provide therapy recommendations to the team.
  • Educate and consult with caregivers, families, and direct support professionals on therapeutic strategies and carryover activities.
  • Maintain accurate and timely documentation in accordance with DDD, AHCCCS, and Medicaid requirements.
  • Monitor progress, revise goals, and support discharge planning as appropriate.
  • Stay current with Arizona OT licensure, DDD policies, and best practices in developmental disability services.
  • Comply with AHCCCS Policy 310-X regarding scope, frequency, and medical necessity of OT services.

FamAli LLC is a Qualified Vendor contracted with Arizona’s Division of Developmental Disabilities (DDD). We provide person-centered therapy and support services to individuals with developmental disabilities across Arizona. Our mission is to promote independence, inclusion, and quality of life through evidence-based and culturally responsive care.

As an Occupational Therapist, you will deliver services in alignment with AHCCCS and DDD standards, collaborating with interdisciplinary teams, families, and DDD Support Coordinators.

Key Responsibilities
  • Conduct DDD-compliant occupational therapy evaluations, including functional, sensory-motor, fine motor, self-care, and adaptive equipment assessments.
  • Develop and implement Individualized Treatment Plans (ITPs) based on member needs, ISP goals, and regulatory guidelines.
  • Provide direct OT services in home, community, or facility-based settings.
  • Recommend and train on assistive technology, adaptive devices, and environmental modifications.
  • Participate in Individual Support Plan (ISP) meetings and provide therapy recommendations to the team.
  • Educate and consult with caregivers, families, and direct support professionals on therapeutic strategies and carryover activities.
  • Maintain accurate and timely documentation in accordance with DDD, AHCCCS, and Medicaid requirements.
