Job summary

The Minnesota Masonic Home in Bloomington, MN is seeking an Occupational Therapist to evaluate, plan, and deliver quality OT services for residents and patients.

You will develop individualized treatment plans, collaborate with nursing and therapy teams, document progress, and support daily living skills to improve residents' independence and quality of life.

Applicant must hold an active Minnesota OT license and maintain professional standards in a compassionate, resident-centered environment.

Qualifications

• Current MN Occupational Therapy license.

Responsibilities

• Develop and implement individualized treatment plans based on evaluation findings and established goals.
• Provide therapeutic interventions to improve daily living skills, mobility, coordination, strength, and independence.
• Monitor and document client progress and modify treatment plans as appropriate.
• Educate clients, families, caregivers, and other members of the care team regarding treatment plans and strategies.
• Collaborate with nurses, therapists, caregivers, and other professionals as appropriate.
• Maintain accurate, timely, and compliant clinical documentation.
• Participate in care planning, team meetings, and quality improvement activities.
• Follow applicable professional, regulatory, and organizational standards.
