Job summary

Granite Peak Home Health in Billings, MT seeks a licensed Occupational Therapist to provide in-home therapy to adults and seniors recovering from illness, injury, or surgery. This role focuses on patient-centered care and independent assessment in patients’ homes.

You will perform OT evaluations, develop and implement treatment plans, and help patients improve ADLs and IADLs while ensuring Medicare compliance and accurate EMR documentation.

Qualifications

• Active Montana OT license (MT OT)
• Degree from an accredited OT program
• Minimum 1 year in home health, skilled nursing, rehab, or acute care (preferred)
• Knowledge of Medicare guidelines and OASIS documentation (preferred)
• Strong skills in patient assessment, care planning, and clinical decision-making
• Valid driver’s license and ability to travel to patient homes
• Current CPR/BLS certification

Position Summary

We are seeking a licensed Occupational Therapist (OT) for a home health position. This role focuses on adult and geriatric patient care, providing in-home therapy services to individuals recovering from illness, injury, or surgery.

This is a great opportunity for an OT looking for flexible scheduling, autonomy, and meaningful one-on-one patient care in a home health setting.

Key Responsibilities

• Perform occupational therapy evaluations and OASIS assessments (if applicable)
• Develop and implement individualized treatment plans
• Provide skilled therapy services in patients' homes
• Improve patients' ability to perform activities of daily living (ADLs) and instrumental ADLs (IADLs)
• Recommend adaptive equipment, home modifications, and fall prevention strategies
• Collaborate with an interdisciplinary healthcare team (RN, PT, SLP, caregivers)
• Complete accurate EMR documentation in compliance with Medicare and home health regulations
• Monitor patient progress and adjust care plans accordingly
