Flagship Therapy in Hampden Hills, Aurora, CO, is seeking a Full Time Occupational Therapist to join our in-house therapy team. You will have opportunities for training in Think Thin/dysphagia, Abilities Care and dementia, stroke and neuro rehab, and respiratory recovery programs.

We offer a flexible full-time schedule with work-life balance and pay between $44 and $50 per hour based on experience. Eligibility for benefits includes medical, dental, vision, 401k, PTO, CEUs, tuition reimbursement,

Qualifications

• Licensed Occupational Therapist eligible to practice in CO.
• Open to applicants of all experience levels.
• Experience in post-acute and geriatric rehab preferred.
• Excellent communication and teamwork skills.

Responsibilities

• Screen and evaluate patients with cognitive and/or ADL impairments and develop care plans.
• Use standardized assessments and evidence-based practices to guide interventions.
• Provide rehabilitative, skilled and medically necessary treatment per regulatory standards.
• Perform ADL retraining, therapeutic activities, exercises, modalities, and use of assistive devices.
• Monitor progress and adjust treatments in the clinical record per requirements.
• Document discharge summaries per requirements and communicate with the health team.

Hampden Hills is a is a post-acute care facility with long-term care, short-term care, and geriatric outpatient services provided. We are seeking a Full Time Occupational Therapist to join our in-house therapy team. As part of this amazing team of clinicians, you will have opportunities for training and continuing education in Think Thin/dysphagia, Abilities Care and dementia, stroke and neuro rehabilitation, and respiratory recovery programs.

• Effectively screens/evaluates patients with cognitive and/or ADL physical functioning disorders and develops appropriate plan of care following all regulatory and clinical practice standards.
• Utilizes standardized assessments and evidence-based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment intervention to patients with cognitive and/or ADL physical functioning disorders per regulatory and clinical practice requirements.
• Administers various procedures as part of the rehabilitation plan including but not limited to: ADL retraining; therapeutic activities; therapeutic exercises; modalities; use of supportive and assistive devices; etc.
• Assesses patient’s progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.
• Evaluates treatment outcomes for effectiveness. Services are modified in relation to patient evaluation and referrals are made in relation to outcomes.
• Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to the physical disorders.
• Effectively communicates with supervisor and other health team members regarding patient progress, barriers, and treatment plans.
• Provides comprehensive discharge summary of services provided per regulatory and clinical requirements.
