We are seeking a licensed, compassionate, and patient-focused Occupational Therapist (OT) to join our rehabilitation team and provide high-quality, skilled therapy services to residents in accordance with physician orders and individualized care plans.
This is an excellent opportunity for an OT who is passionate about patient-centered care, clinical excellence, and helping residents regain function, increase independence, and improve their overall quality of life.
Evaluate residents and develop individualized occupational therapy treatment plans based on clinical needs and physician orders.
Provide skilled occupational therapy services in accordance with established care plans.
Monitor resident progress and modify treatment approaches as clinically appropriate.
Complete accurate and timely documentation, including evaluations, treatment notes, progress reports, and discharge summaries.
Collaborate with nursing, therapy, and interdisciplinary team members to support positive resident outcomes.
Participate in care planning, utilization review meetings, interdisciplinary conferences, and other clinical meetings as assigned.
Educate residents, families, and staff regarding therapy goals, treatment plans, safety, and strategies to promote independence.
Maintain established productivity, documentation, and quality standards.
Support regulatory compliance and survey readiness.
Promote resident safety, dignity, independence, and overall well-being.
Graduate of an accredited Occupational Therapy program.
Current and active state license as an Occupational Therapist.
Long-term care or skilled nursing experience preferred.
Strong clinical documentation and communication skills.
Excellent interpersonal, organizational, and time-management skills.
Ability to work effectively within an interdisciplinary healthcare team.

Ability to stand, walk, bend, and assist residents throughout the workday.
Ability to safely lift, transfer, and support residents during therapy activities.
