Job summary

The Miriam Hospital, part of Brown University Health, seeks an experienced RN for a discharge planning leadership role in Providence, RI. You will coordinate care with medical staff, case management, and utilization management to facilitate timely discharges for observation patients.

Responsibilities include supervising Case Management staff, leading daily rounds, and communicating discharge plans to patients and families while ensuring compliance with federal and state regulations.

Qualifications

• RN license in Rhode Island with RI Board of Nursing.
• Bachelor's Degree required.
• Certification in Case Management by a national organization preferred or to be obtained within 1 year of hire.
• BLS Certification required.

Responsibilities

• Partner with medical staff, case manager, utilization manager, and other team members to facilitate discharge plans for observation patients.
• Identify high-risk patients and assist in timely discharge planning.
• Coordinate communication between case management and utilization management.
• Provide clinical supervision to Case Management staff.
• Participate in 24/7 coverage for clinical and scheduling issues.
• Inform care team about insurance qualifiers affecting discharge plans.
