Job summary

Qualifications

• Licensure as Registered Nurse in Rhode Island or in accordance with the Nurse Licensure Compact.
• Bachelor’s Degree required.
• Certification in Case Management by a national organization preferred or to be obtained within 1 year of hire.
• BLS Certification required.

Responsibilities

• Partners with medical staff and case management team to facilitate discharge plans for observation patients.
• Identifies high-risk patients and assists in timely discharge planning.
• Collaborates with utilization management to ensure communication between teams.
• Participates in daily observation rounds to develop and implement discharge plans.
• Supervises Case Management staff as needed and participates in system-wide coverage.
