Job summary

Qualifications

• Rhode Island RN license required.
• Bachelor's degree in Nursing.
• Certification in Case Management preferred or to be obtained within 1 year of hire.
• BLS certification required.

Responsibilities

• Partner with medical staff, case management, utilization management and other care-team members to facilitate discharge plans for observation patients.
• Identify high-risk patients and assist in timely discharge planning.
• Coordinate communication between case management and utilization management.
• Provide supervision and direction to case management staff as needed.
• Participate in system-wide coverage and department improvement activities.
