Inova Health is seeking a dedicated Licensed Practical Nurse (LPN) 2 for a full-time day position at our Alexandria location. The role runs Monday–Friday, 8:00am–4:30pm, with responsibilities including care coordination and medication administration in collaboration with physicians and the healthcare team.

Required qualifications include a Vocational/Technical Diploma in Practical Nursing, Virginia LPN license, 2 years hospital/clinic experience, and BLS certification.

Qualifications

• Vocational/Technical Diploma in Practical Nursing.
• Licensed Practical Nurse in the Commonwealth of Virginia.
• 2 years of nursing practice in a hospital or clinic setting.
• BLS from the American Heart Association.

Responsibilities

• Coordinates referrals for patient chronic care conditions and follow up care as indicated by the initial care plan of the specialist and/or primary care physician.
• Administers prescribed oral and intramuscular medications, therapies and treatments following practical nursing standards and under the direct supervision of the physician.
• Effectively communicates with healthcare team members, patients and families; Supports healthcare team members to achieve team goals and assigns appropriate tasks to care assistants.
• Maintains and reviews patient medical records, notes and physician orders; Documents all patient care activities in accordance with documentation standards and legal requirements for medical facilities.
