The ideal candidate will review OASIS assessments, audit Plans of Care, verify orders, and ensure documentation supports medical necessity and skilled services. They will identify issues, collaborate with clinicians, and contribute to QA and program improvements.

Qualifications

Responsibilities

• Review patient charts to ensure accuracy and completeness.
• Audit Plans of Care to reflect patient needs and skilled services.
• Review physician orders for accuracy and compliance.
• Ensure documentation supports medical necessity and home health services.
• Identify missing or unclear documentation and collaborate with clinicians to correct it.
• Monitor for compliance issues and escalate as needed.
• Contribute to QA/QAPI initiatives and education for the team.
