Concordia Visiting Nurses is a Medicare-certified home health agency seeking a Clinical Documentation and Workflow Auditor to join our Valencia, PA team. You will oversee OASIS data, perform audits for regulatory compliance, and collaborate with Quality and Education teams to drive improvements in patient care.

Qualifications

• Minimum of two (2) years of home health experience with progressive responsibilities.
• OASIS certification is recommended; if not currently certified, must obtain within one year.
• Knowledge of agency policies, federal and state regulations.
• Strong interpersonal, communication, and organizational skills with evidence of leadership activities.

Responsibilities

• The Oasis Workflow Registered Nurse (RN) oversees quality measures and data to ensure submitted information aligns with OASIS guidance and regulatory requirements.
• Conduct reviews of OASIS assessments in accordance with best practice guidelines; evaluate outcomes using data mining software.
• Improve ancillary services by analyzing assessments to foster positive patient outcomes.
• Detect inconsistencies and errors in clinical documentation and report findings to Quality/Education teams.
• Participate in Quarterly Quality Record Reviews.
• Work with the education team to provide quarterly OASIS training and participate in quality/rehospitalization meetings.
