Job summary

General Job Description: The Nutrition Assessment Manager oversees the Nutrition Assessment team and processes at The Senior Alliance for the Home Delivered Meals program, which includes managing a team of staff, conducting nutrition assessments as needed, and managing assessment scheduling and data collection. The Nutrition Assessment Manager must be able to work with older adults, caregivers, and individuals living with disabilities. The Nutrition Assessment Manager schedules home delivered meals nutrition assessments that occur in participant homes with Nutrition Assessors. The Nutrition Assessment Manager will conduct nutrition assessments when required, which could include more assessment requests than the Nutrition Assessors can manage or if there are staff shortages. This position requires exceptional interpersonal skills, a detail-oriented approach, and the ability to provide tailored recommendations for services. The Nutrition Assessment Manager must be flexible and be able to problem solve when last minute schedule changes occur.

Physical Requirements:

  • Regularly required to sit for extended periods of time.
  • Regularly spend long hours using office equipment primarily telephones and computers.
  • Regularly work on repetitive tasks and motions.
  • Ability to drive a vehicle throughout The Senior Alliance service area, including to client homes.
  • Ability to lift up to 40 pounds.
  • Stand, walk, reach, and bend.
  • Ability to walk on uneven surfaces including outdoor sidewalks.

Duties:

  • Manage staff including timecard management, performance reviews, scheduling, recruiting, training, etc.
  • Meet with each employee regularly to oversee work, provide consultation, and assess skillset.
  • As needed, travel to home delivered meals program participants, including program applicants, homes within The Senior Alliance service area to perform in-person assessments, reassessments, and evaluations.
  • As needed, conduct detailed evaluations to understand the participants’ needs, preferences, and circumstances.
  • Maintain accurate and up-to-date records of all assessments, visits, and interactions.
  • Manage home delivered meals nutrition assessment process, including receiving applications, contacting nutrition program applicants to introduce the program, conducting initial assessment questions for intake, scheduling in-person assessments, managing wait-lists, and enrolling eligible participants into the program.
  • Provide education and guidance regarding the home delivered meals program, including requirements.
  • Implement and comply with state operating standards for nutrition programs.
  • Develop and provide training plan for The Senior Alliance home delivered meals nutrition assessment team.
  • Adhere to all organizational policies, confidentiality requirements, and ethical standards.
  • Assign daily schedules for Nutrition Assessors, ensuring they have their assignments at least one day in advance.
  • Must be able to make unexpected changes to the schedule and communicate changes to Nutrition Assessors in a timely manner.
  • Ensure daily confirmation of the next business day's appointments.
  • Complete and submit reports as requested.
  • Initiate incident reports when appropriate and complete with management.
  • Create agendas and run monthly Nutrition Assessment team meetings and training.
  • Develop, prepare, and produce accurate programmatic and data reports, as required.
  • Maintain accurate participant records in program databases.
  • Track assessment schedule to ensure six-month and annual assessments are conducted on time.
  • Contribute to the continuous improvement of services.
