Quality Home Health, Hospice & Palliative Care in Utah is seeking an experienced Assistant Director of Nursing (ADON) to provide leadership and clinical guidance across our growing Home Health and Hospice team.

As ADON, you will work with the Director of Nursing to oversee day-to-day operations, mentor RN Case Managers, LPNs, and aides, ensure regulatory compliance, and lead quality improvement initiatives while maintaining a patient-centered culture.

Qualifications

• Current Registered Nurse license in Utah.
• BSN preferred.
• Minimum of two years of nursing experience or equivalent.
• Leadership or supervisory experience preferred.
• Home Health and/or Hospice experience preferred.
• Familiarity with OASIS and Home Health documentation desirable.
• CPR certification.
• Current driver’s license and reliable transportation.

Responsabilités

• Provide day-to-day clinical leadership, guidance, and support to RN Case Managers, LPNs, aides, and other clinical staff.
• Serve as a clinical resource for staff questions, concerns, or patient condition changes.
• Help clinicians establish priorities, problem-solve challenging situations, and ensure patient needs are addressed.
• Review referrals and assist with determining services, staffing, and clinician assignments.
• Support coordination of care across the clinical team.
• Monitor clinical documentation for accuracy, timeliness, and regulatory compliance.
• Assist with clinical chart review and quality assurance activities.
• Review OASIS documentation for accuracy and provide education as needed.
• Ensure compliance with Home Health and Hospice regulations and agency policies.
• Lead and participate in clinical meetings and interdisciplinary discussions.
• Support staff education, competency development, and ongoing clinical training.
• Provide coaching and feedback to help team members grow professionally.
• Assist with orientation and training of new clinical employees.
• Participate in interviewing and selecting new clinical team members.
• Monitor staffing needs and clinical workloads; communicate needs to leadership.
• Assist with employee evaluations and performance management.
• Participate in quality improvement and QAPI initiatives.
