Qualifications

• Must have a current Connecticut state license to practice as a Licensed Practical Nurse (LPN).
• Completion of an approved LPN educational program is required.
• A high school diploma or GED (or equivalent) is necessary.
• At least 6 months of experience as an LPN or in a similar role is essential.
• Candidate must be able to lift up to 50 lbs.

Responsibilities

• Provide post-acute LPN nursing care to residents with varying medical needs.
• Track vital signs, physical condition, and mental acuity of residents.
• Complete intakes of residents, documenting medical histories and health concerns.
• Assist with medications including administration, verification, and monitoring.
• Keep track of residents' medical records and charts.
• Help maintain a safe and comfortable environment for residents.
• Ensure proper storage and inventory of medical supplies.
• Assist with immunizations, IVs, and phlebotomy procedures.
• Follow and adhere to care plans for residents and applicable regulations.
• Support training and orientation of new clinicians.
