Job summary

Mass Digital Health is seeking a Licensed Practical Nurse (LPN) in Hamden, CT. The position offers a pay range of up to $57 per hour, flexible scheduling, and W-2 benefits. Responsibilities include providing nursing care, tracking resident conditions, and managing medical records.

Qualified candidates should possess a current Connecticut LPN license, completion of an approved LPN program, and at least 6 months of experience.

Qualifications

• Current Connecticut state license to practice as a Licensed Practical Nurse (LPN).
• Completion of an approved Licensed Practical Nurse (LPN) educational program.
• High school diploma or GED (or equivalent).
• 6 months experience as an LPN (or similar role).
• Ability to lift 50 lbs.

Responsibilities

• Providing post-acute LPN nursing care to residents with varying medical needs.
• Tracking vital signs, physical condition, and mental acuity of residents.
• Completing intakes of residents, including documentation of medical histories.
• Assisting with medications, including administration and monitoring.
• Keeping track of medical records and charts of residents.
• Helping maintain a safe, comfortable, and pleasant environment.
• Ensuring proper storage and inventory of medical supplies.
• Setting up and assisting with immunizations, IVs, and phlebotomy.
• Following care plans for residents and applicable regulations.
• Supporting training and orientation of new clinicians.
