Job summary

Health & Human Services Comm in Texas seeks a LTCR Nursing Facilities Social Services Surveyor to conduct on-site inspections and investigations across long-term care facilities. The role requires travel up to 80% and report-writing responsibilities to assess compliance with state and federal regulations.

Eligible candidates will have SMQT certification or be willing to obtain it within 12 months, and be licensed as a social worker by the Texas State Board of Social Work Examiners.

Responsibilities

• Conduct on-site licensure inspections and investigations.
• Perform complaint and incident investigations and annual comprehensive surveys.
• Complete inspection reports detailing deficiencies and corrective remedies.
• Communicate findings clearly to stakeholders internally and externally.
• Conduct exit conferences with facility staff regarding findings.
• Visit facilities to verify correction of cited violations and perform QA reviews.
