Job summary

Mary’s Meadow at Providence Place, part of Trinity Health, seeks a Quality of Care Coordinator to lead daily clinical operations, ensure high-quality resident-centered nursing care, and drive the QA/PI program in a home-like setting.

This role manages staffing, budget oversight, infection control, and regulatory compliance with an on-site presence and monthly nursing duties. Competitive compensation offered.

Qualifications

• BSN and Valid RN license in the state of Massachusetts required.
• Three (3) years of Supervisory experience preferred; Long-term care experience preferred.
• Knowledge of Joint Commission and DPH standards preferred.
• Demonstrates superior written and verbal communication and presentation skills.
• Proven ability to develop and implement budget.

Responsibilities

• Performs daily rounds to assess the resident and evaluate the effectiveness of the clinical care plan.
• Works in the capacity of a licensed nurse for one shift per month to evaluate clinical care delivery systems.
• Assures the effectiveness of the infection control program.
• Responsible for interviewing, hiring, orientation, development, corrective action and evaluation of staff to ensure quality care to residents.
• Reviews staff compliance with in-service requirements and supports staff development.
• Leads the care planning process and ensures holistic resident-centered care.
• Monitors QI indicators and coordinates quality improvement with the clinical team.
• Manages admissions; creates and reviews key EMR reports.
• Creates policies and procedures with the clinical team.
• Reviews emergency protocols and conducts drills; participates in regulatory meetings.
