Job summary

Capital Health Care Network in Ohio seeks a Director of Nursing (DON) to oversee coordination, delivery, and management of patient care and services in our home health program.

This leadership role requires a proven background in supervision, regulatory compliance, and clinical guidance to clinicians to ensure excellent patient outcomes and top-tier customer service. You will drive quality improvement and policy initiatives within the agency.

Qualifications

• RN with current licensure to practice professional nursing in the State.
• Graduate of accredited school of nursing.
• Prefer management or supervisory experience in a health care setting, preferably home care.
• Must be a licensed driver with automobile insured and in good working order.
• Maintains current CPR certification.
• Demonstrates excellent observation, verbal and written communication skills.

Responsibilities

• Reviews case referrals and assigns clinicians to cases.
• Evaluates services provided by clinicians and guides performance for quality home care.
• Reviews patient diagnoses, medications, procedures and clinical course.
• Assists in setting goals and developing care plans.
• Holds case conference meetings to coordinate care.
• Oversees staff and provides direction on performance improvement strategies.
• Ensures quality of patient care and customer service.
• Performs record reviews and communicates findings to agency personnel.
• Assists in interviewing and orientation of new staff.
• Supports in-service and continuing education programs.
• Contributes to policy formulation and strategic planning.
