Job summary

Holy Trinity seeks an experienced Director of Nursing to lead its 113-bed skilled nursing facility. You will direct the nursing department, guide clinical policies, and ensure CMS 5-Star standards across all shifts while partnering with physicians, residents, and families to deliver resident-centered care.

Ideal candidates bring 3–5 years of leadership in long-term care, RN licensure, and a proven track record in quality improvement and team mentorship in a regulated environment.

Qualifications

• RN license in the state of practice.
• 3–5 years of progressive nursing leadership in SNF/long-term care.
• In-depth CMS survey process knowledge and health codes.
• Demonstrated leadership and mentoring abilities.
• Excellent verbal and written communication with families, staff and surveyors.

Responsibilities

• Direct, plan, and oversee daily nursing operations to ensure compliance with regulations.
• Lead recruitment, onboarding, retention, and ongoing mentorship of nurses and CNAs.
• Monitor clinical outcomes, drive QAPI initiatives, and ensure survey readiness.
• Review and update clinical policies and infection control protocols.
• Partner with Medical Director and care team to create tailored care plans.
• Manage scheduling, supplies, and departmental budgets.
