Friendship-Community in Lititz, PA, seeks a Nursing Consultant to provide medical support in meeting health care needs of Individuals in the Friendship Community Waiver homes.

You will perform nursing tasks, monitor health, communicate medical status to managers and staff, attend medical appointments, and train team members on health care issues. The role requires PA nursing license and travel to service locations.

Qualifications

  • Must hold a current Pennsylvania nursing license (RN or LPN).
  • Experience with Individuals with Intellectual Disabilities preferred.
  • Computer skills in Microsoft Office are required.
  • Reliable private transportation to drive to service locations is required.

Responsibilities

  • Demonstrates a respectful positive attitude that promotes service excellence to Individuals, fellow Team Members, families and/or the general public.
  • Perform and provide direction to nursing tasks such as medication administration and health monitoring.
  • Inform Residential Manager and Direct Support Professionals of medical status through written and verbal communication.
  • Assist with reporting medical changes to family, day program, and Support Coordinator when requested.
  • Ensure health-related case notes are informative and describe conditions, care rendered, and responses.
  • Advise on special diets relative to health conditions.
  • Attend and/or prepare for medical appointments as requested.
  • Identify and ensure treatment of communicable diseases; report infectious conditions.
  • Provide training for Team Members and Individuals regarding health care issues.
  • Attend meetings and committees as agreed; compile Lifetime Medical History Summary and update annually.

Job description

The Nursing Consultant provides medical support in meeting the health care needs of Individuals in the Friendship Community Waiver homes.

V. Responsibilities:
  • Demonstrates a respectful positive attitude that promotes service excellence to Individuals, fellow Team Members, families and/or the general public.
  • Perform and provide direction to nursing tasks such as: medication administration, medical aspects for the health and wellness of the Individuals, and being available on-call for consultation regarding health concerns.
  • Communications
    • Inform the Residential Manager and Direct Support Professionals of Individuals’ current medical status and other related matters through written and verbal communication.
    • Assist the Residential Manager or Residential Coordinator with reporting medical changes to family, day program, and Support Coordinator when requested.
  • Ensure that all health-related case notes are informative and descriptive of conditions present, care rendered, including Individual’s response.
  • Advise on special diets relative to health conditions.
  • Attend and/or prepare for medical appointments as requested.
  • Identify and ensure treatment of communicable diseases. Report all infectious conditions to the Nursing Services Coordinator.
  • Assist with a variety of medical orientations for Team Members.
  • Provide training for Team Members and Individuals regarding health care issues.
  • Attend various meetings, including Nursing Services Committee, Team Meetings, in-services and other committees as agreed upon.
  • Compile a Lifetime Medical History Summary for each Individual and update annually as assigned.
VII. Work Schedule:

This is a Full-time/Max Part-time/Part-time position which requires flexibility in schedule. A majority of the hours are Monday-Friday daytime. Some evening and weekend hours are required, as needs arise.

VIII. Education / Experience Requirements:
  • Must hold a current Pennsylvania nursing license. Either an RN or LPN is acceptable.
  • Experience in working with Individuals with Intellectual Disabilities is preferred.
  • Computer skills in Microsoft Office are required.
  • Reliable private transportation and ability to drive to various service locations required.
