Job summary

Incompass Human Services, Inc. is seeking a Licensed Practical Nurse to oversee nursing care and medical documentation for program participants, under RN supervision.

The role requires an active MA LPN license, at least one year working with the elderly, children, or developmentally disabled individuals, and strong communication skills. The position is on-site with a schedule Sunday-Thursday 9:00am-5:00pm.

Qualifications

Responsibilities

• Monitors the health of program participants and provides nursing care.
• Manages and documents medical information, referrals, reports, and medication administration.
• Completes medical record audits and manages medical tracking systems.
• Assesses individuals with major medical issues and oversees self-medication processes.
• Provides staff training on medical needs and emergency procedures.
• Coordinates care between Residential and Day Habilitation for continuity of care.
• Acts as a mandated reporter under the Disabled Persons Protection Commission.

Summary

Monitors the health, safety and well being of participants in the Residential system. Incumbent is responsible for overseeing and managing the medical and health care of program participants including providing direct nursing care. Incumbent reports issues to appropriate persons and handles emergencies according to Incompass Human Services and other applicable agencies guidelines. Incumbent must comply with applicable regulatory documentation and standards in the performance of nursing duties.

This position meets the regulatory requirements set forth in 130 CMR 419.426(G) for Licensed Practical Nurses and is directly supervised by an RN as required.

This is a bargaining unit position.

Essential Job Functions

• Monitors the health of program participants, including preventative care, and provides professional technical assistance regarding nursing care and medical treatment as needed.
• Responsible for management and documentation as it relates to medical information, referrals, reports, medication administration and data collection for use in annual planning, goal setting and intervention strategies.
• Responsible for the completion of medical record audits and management of medical tracking systems.
• Assesses individuals as assigned on a regular basis and provides management for individuals with major medical issues.
• Provides oversight and assistance with the self-medication assessment process.
• Provides oversight of MAP and for the administration of medical treatments and staff training as needed including annual mandated staff trainings. Administration is done in accordance with Nurse Practioner’s Act and DDS and MAP regulations.
• Provides assistance when needed regarding medication supplies and medical supplies as needed.
• Orients and trains staff in understanding participants’ medical needs and in following prescribed programs and plans. Communicates with and trains staff to complete procedures as appropriate.
• Provides leadership and support to team during medical crisis.
• Ensures professional communication with families, DSS, clinicians and others for assigned participants.
• Provides guidance to staff regarding medical emergencies and informs supervisor of all emergency treatments or 911 calls made.
• Communicates with medical providers regarding major issues.
• Completes nursing notes and documentation for encounters with individuals.
• Attends meetings as assigned
• Ensures the coordination between Residential and Day Habilitation program for continuity of care.
• Serves as a mandated reporter as required by the Disabled Persons Protection Commission.
• Processes all human rights issues, incident and accident reports in a timely manner according to agency policy/procedure. Ensures the human rights of program participants are safeguarded in all interventions.
• Responsible for knowing and following agency, departmental, program and site policies and procedures as well as the bargaining unit personnel policies handbook and the collective bargaining agreement.
• Fosters and maintains positive relationships with families, participants and staff and informs supervisor of major and/or unresolved issues.
• Acts in a manner consistent with organizational values, program mission, and service delivery goals. Serves as a resource and role model for staff.
• Completes other duties as assigned by supervisor as they relate to the duties in this job description.

Certificates, Licenses, Registrations

• Current Adult CPR certification.
• PAC or other form of current agency approved restraint certification must be acquired during the first six months on the job.
• Employee is required to remain current with Adult CPR certification, PAC/restraint certification and LPN license. Nurse must maintain a clean record and not have any pending discipline actions with the Commonwealth of Massachusetts or job status will be reviewed with the outcome to be decided by a management team.
