Job summary

Infinite Medical P.C. is seeking an on-site Nursing Care Coordinator (RN or LPN) to review chart documentation and support continuity of care for nursing facility patients.

You will verify records for CMS compliance, coordinate with HR and clinical teams, and participate in mandatory trainings while using PCC, MatrixCare, SigmaCare, and other EMR tools. The role offers a daytime schedule (Mon–Fri, 9:00 AM–5:30 PM) with benefits and a $1,000 sign-on bonus.

Qualifications

• RN or LPN licensure required.
• Experience with geriatric care and care coordination.
• Strong documentation and chart review abilities.
• Excellent written and verbal communication.

Responsibilities

• Review patient records and logs to ensure CMS compliance.
• Coordinate with HR, Operations, and Clinical teams.
• Participate in mandatory operational and educational meetings.
• Maintain proficiency in primary EMR platforms.
• Support care management and care transitions.
