Sapphire at Rivers Edge in Sheridan, OR is seeking a dedicated Nursing Assistant to support residents with daily living activities and help achieve their healthcare goals. You will collaborate with the nurse supervisor to monitor changes in condition and ensure high-quality, compassionate care.

Responsibilities include routine nursing care, accurate charting, maintaining confidentiality, and effective communication with staff, residents, and families.

Qualifications

• Must be 18 years of age.
• Proof of Nursing Assistant program completion certificate.

JOB SUMMARY:

As a Nursing Assistant (NA) withSapphire you will assist residents with daily living activities while supporting each individual in reaching their healthcare goals. Sapphire will provide the training and resources to keepour Nursing Assistants (NA) engaged and maintain top quality skills.

RESPONSIBILITIES:

• Provideresidents with routine daily nursing care and services
• Chartall appropriate documentation
• Maintainpatient confidentiality
• As a Nursing Assistant (NA) you will workwith nurse supervisor to monitor any changes in condition
• Communicateboth orally and written with staff, residents, family members, etc.
• Fostera supportive, compassionate environment for our residents and their families
• Adheres to the policies and procedures of the facility
• Otherduties as assigned

REQUIREMENTS:
