Qualifications

• High school diploma or equivalent preferred.
• Ability to read and interpret documents, write routine reports, and communicate effectively.
• Proficiency with Outlook, Excel and Word required.

Responsibilities

• Greet patients upon admission and assist as requested.
• Assist patients with meals (tray delivery, feeding, etc.).
• Record patient intake and output as requested.
• Identify signs of skin breakdown and assist with skin care.
• Perform bedmaking and hygiene tasks as trained.
• Provide post-meal care and routine patient assistance.
• Assist with walking and mobility as directed.
• Document and report patient condition changes to nursing staff.
