Job summary

The Nurse Manager position at Bonne Vie Skilled Nursing in Port Arthur, TX oversees patient care delivery in alignment with physician orders and accepted standards of practice. The role ensures direct patient care and supervises the direction of care to maintain high-quality outcomes.

Responsibilities include rounds every 4 hours, reviewing daily records, coordinating with the Interdisciplinary team, and ensuring accurate documentation.

Qualifications

• Graduation from an accredited Nursing School and a current and valid LVN/LPN license in the applicable State.
• Minimum of one-year supervisory experience in a long-term or health care setting is preferred.
• Ability to read, write, and speak the English language.

Responsibilities

• Monitor and review daily documentation for accuracy and intervention.
• Make rounds on all patients at a minimum of every 4 hours and communicate findings to the charge nurse.
• Review the 24-hour report book for changes in condition and ensure timely assessment and intervention.
• Review incident reports and ensure proper nursing documentation and notification.
• Review medical records on discharge for accuracy and follow-up care planning.
• Enter physician orders daily into the clinical software system and verify for accuracy.
• Complete MDS assessments, RAPs, Care Plans and TILE assessments as required.
• Attend weekly care plan meetings with the Interdisciplinary team and related parties.
• Maintain patient safety and perform other duties as assigned.
