Responsibilities

• Coordinate care with multidisciplinary teams to ensure a safe, quality patient experience.
• Act as liaison to assign beds, meet care needs, and execute treatment plans.
• Communicate with physicians and care teams to progress patients toward discharge.
• Maintain accurate patient status in EPIC and update in real time.
• Lead family meetings and plan of care discussions.
• Support financial entitlements and referrals as needed.
• Document Care Management activities per SOPs; meet metrics.
• Participate in or lead required meetings.

Job Details

Our Nurse Transition Coach will:

• Collaborate and coordinate with multidisciplinary teams to facilitate a safe quality experience for patients through the continuum of care
• Act as a Liaison between all patient care services to ensure appropriate bed is assigned, patient care needs are met and treatment plans are executed in the appropriate level of the continuum.
• Communicate/collaborates daily with physicians, unit and support team ( MSW, CDI, PT, etc.) to progress each patient's care towards discharge and resolve barriers to discharge
• Escalate complex patient care coordination needs to appropriate support team member
• Serve as an expert resource in patient flow, status and progression and provides ongoing education to staff, physicians, patients, and their significant others
• Ensure designated software/EPIC reflects accurately the patient status and is updated in real time as needed
• Reviews Target LOS values and working DRG daily and at points of patient condition changes
• Facilitate family meetings and plan of care discussions related to supportive care, healthcare proxy and guardianship
• Work collaboratively with care team to facilitate financial assistance, entitlements and medication assistance as requested. Provide community resource information as requested and facilitates or completes referrals as directed
• Perform the Care Management functions and documentation as per orientation and competency documents
• Meet deadlines and exceeds metrics identified for the Facility/ Unit through implementation of facility policies and Standard Operating Procedures (SOPs)
• Participate or lead meetings as required

What you'll bring:

• 3+ Years in-patient acute care clinical RN experience required
• Experience in Care Management preferred
• 2+ Years progressive nursing leadership experience preferred

• Education: Graduate of an accredited School of Nursing required
  • Bachelors of Science in Nursing (BSN) degree preferred
• Licensure: New York Registered Nurse (RN) License & Registration

• Certification: Related specialty certification preferred; Case Management Certification preferred; PRI Certification preferred

• Skills: Beginner proficiency of Outlook, Word and Excel is required. Beginner proficiency of Power Point is preferred.

Salary Range

USD $120,696.00 - USD $152,839.00 /Yr.
