Signature HealthCARE in Memphis, TN is seeking a dedicated LPN/LVN to provide direct nursing care for residents and support daily living activities. You will supervise nursing assistants, administer medications, and monitor residents’ status in collaboration with the care team.

The role requires an active LPN/LVN license, CPR/BLS certification (preferred), and at least 1 year of nursing experience, with long-term care experience preferred.

Qualifications

• Active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one year of clinical nursing experience; long-term care experience preferred.

Responsibilities

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
• Supervise and direct nursing assistants and medication aides during the assigned shift per policies and regulations.
• Monitor and observe residents’ status; notify physician and responsible party of changes.
• Develop, implement, and review individualized resident care plans with the interdisciplinary team.
• Complete accurate clinical documentation per policy.
• Coordinate resident admissions, transfers, and discharges according to procedures.
• Participate in quality assurance activities, staff education, and regulatory surveys as needed.
• Perform other duties as assigned by the Director of Nursing.
