Job summary

Horizon House, Inc. is seeking staff to provide assistance with personal care and activities of daily living in our community-based programs. You will support residents with daily routines and household tasks under the supervision of a Nurse/Home Coordinator.

The role emphasizes teamwork, independent work, and strong communication skills to ensure compassionate, compliant care for individuals with diverse needs. Prior care experience is preferred.

Qualifications

• Active RN Student, or Registered Nurse, or Licensed Practical Nurse.
• Prior hospital, nursing home, community health agency, private duty home care experience preferred.
• Good communication skills, both oral and written.
• Ability to work independently and as part of a team.

Responsibilities

• Provide assistance with personal care, activities of daily living, and maintenance of household routines.
• Work under supervision of Nurse/Home Coordinator; follow care plans and safety protocols.
• May be required to possess a valid driver's license depending on location and program needs.

Horizon House, a non-profit organization, has been widely recognized for its services as a provider of community-based behavioral health, intellectual disabled and homeless services in Eastern Pennsylvania and the state of Delaware. Each year, Horizon House provides a continuum of services at over 100 sites to over 4,5000 adults with psychiatric or developmental disabilities, drug and alcohol addictions, and/or homelessness. Purpose:

To provide assistance with personal care, activities of daily living, and maintenance and management
of household routines.

Supervision Exercised: N/A

Qualifications:

Active RN Student/or Registered Nurse/or Licensed Practical Nurse Prior experience in a hospital, nursing home, community health agency, private duty home care or any combination of experience, education or training that would provide the level of knowledge, skill and ability required. Prior experience or interest in the MH/MR field preferred. Good communication skills, both oral and written. Ability to work independently and as part of a team.
Scope:

Requires the ability to lift, carry fold, kneel, reach, stack, stoop and bend which may include direct intervention with residents. Frequent talking and listening. Possess manual dexterity and fine motor skills. May be required to possess a valid driver's license and an acceptable driving record (depending upon facility location and program needs).
