Horizon House, Inc. is seeking a Registered Nurse to lead clinical and administrative functions within the Developmental Services Division. The role focuses on quality of care, staff supervision, and participant education in a community-based setting.

The incumbent will promote outcomes-based nursing practice, supervise Nurses and DSPs, and ensure adherence to nursing standards while coordinating with the Regional Director. Equity and professional development are emphasized.

Qualifications

• Bachelor's degree in Nursing; active RN license in Pennsylvania.
• 3–5 years progressive healthcare management experience.
• Experience in community nursing and/or developmental disabilities desired.
• Experience with electronic health records preferred.
• Excellent interpersonal, communication and organizational skills.
• Proficient use of Microsoft Office Suite for recordkeeping and documentation.
• Valid driver’s license with acceptable driving record.

Responsibilities

• Provide clinical and administrative supervision to Nurses, DSPs and others as assigned.
• Develop, implement and monitor nursing standards of care and quality outcomes.
• Educate participants, DSPs, family and friends on disease management and self-care.
• Assist with system development to improve quality and cost-effectiveness of services.

Horizon House, a non-profit organization, has been widely recognized for its services as a provider of community-based behavioral health, intellectual disabled and homeless services in Eastern Pennsylvania and the state of Delaware. Each year, Horizon House provides a continuum of services at over 100 sites to over 4,500 adults with psychiatric or developmental disabilities, drug and alcohol addictions, and/or homelessness. A pioneer in the field of community-based rehabilitation, Horizon House is widely recognized for its innovative approach to the delivery of services in a community setting.

Currently, we are seekingRegistered Nurse to work in our Developmental Services Division. This is a split position with zip codes (19468, 19342, 19601)

Purpose:

• Promotes quality of healthcare by developing and interpreting nursing philosophies and standards of care; enforcing adherence to nursing best practices; measuring health outcomes against standards; making adjustments –all in a community-based, licensed, setting.
• Promotes participants’ independence by establishing participant care goals; teaching and counseling participants, Direct Service Professionals (DSPs), family and friends and reinforcing their understanding of disease; medications; medical procedures –to achieve/maintain healthy life styles and self-care skills.
• Maintains and shares knowledge of federal and state healthcare reform.
• Develops, implements and reviews systems to insure quality and cost effectiveness of operations affecting the health, behavioral healthcare and behaviors of all participants with developmental disability and/or Autism.
  Supervision Received:
• Receives direct supervision from the Regional Director.
  Supervision Exercised:
• Provide clinical and administrative supervision to Nurses, DSPs and others, as assigned.
  Qualifications:
• Bachelor's degree in Nursing; Registered Nurse. Maintain current State of Pennsylvania licensed as RN.
• Three to five (3 - 5) years of progressive healthcare management experience. Experience in community nursing and/or developmental disabilities desired.
• Prefer experience with electronic health records.
• Excellent interpersonal skills.
• Good organizational skills, good communication skills and detail oriented.
• Proficient use of Microsoft Office Suite for recordkeeping, form development, word processing, and experience with data base design and maintenance.
• Possess valid driver’s license with acceptable driving record.
  Scope:
• Requires the ability to lift, carry, fold, kneel, reach, stoop, stack, bend, light lifting and climbing stairs.
• May include direct intervention with participants, to include crisis (medical & psychiatric) intervention in emergency situations.
• Frequent talking and listening.
• Requires flexibility in scheduling
• Good manual dexterity and fine motor
• Required to carry a cell phone during regularly scheduled working hours. As member of essential personnel, may be required to report to work during emergencies including inclement weather
