Responsibilities

• Assists in outpatient clinic by rooming patients and performing duties typically assigned to the Nurse/Desktop.
• Assists physician with patient exams/treatments/procedures during inpatient rounds as needed.
• Coordinates communications with referring physicians, specialists, patients, families and hospital departments to ensure timely information exchange.
• Documents responses to patient history/screening questions for providers during assessments and examinations.
• Maintains computer documentation proficiency for both outpatient and inpatient settings.
• Monitors patients with chronic conditions andEducates for the next level of care to reduce readmission risks.
• Prioritizes physician workload related to inpatients, observation patients, and ED presentations.
• Relays information between providers and clients, including patients, families, pharmacies, nursing homes and insurers.
• Communicates provider advice, orders and results per physician instructions and documentation.
• Responds to department emergencies in Heart Center and Cath Lab with ACLS as needed.
• Reviews patient records for complete documentation and quality metrics compliance.

Nurse Specialist are responsible to prioritize/manage their time/resources as applicable to the needs of the outpatient and/or inpatient settings. They Coordinate communication between Physicians and Medical staff members, other outpatient and inpatient departments/staff, patient and family. They Gather clinical information, results of tests, discharge plans, and changes in patient condition to facilitate patient and staff communication with Physicians.

Responsibilities

• Assists physician with patient exams/ treatments/procedures during inpatient rounds as needed.
• Coordinates communications with referring physicians, specialists, patients, families and hospital departments/staff to facilitate timeliness and thoroughness of information shared.
• Documents responses to patient history/screening questions for use by providers during assessments and examinations.
• Computer documentation proficiency to perform this task on both the outpatient and inpatient setting.
• Monitors patients with chronic conditions and ensures appropriate education and expectations with the next level of care to support health maintenance and reduce readmission risks.
• Prioritizes assigned physician workload related to inpatients, observation patients and patients presenting to the Emergency Department.
• Relays information between provider(s) and client(s), which may include but is not limited to communications with patients, families, pharmacies, nursing homes, consultancy office staff, insurers, and facilities.
• At their physician’s instruction, communicates provider advice, instructions, orders and results, as applicable, including accurate documentation.
• Responds to all department-specific emergencies in the Heart Center and Cath Lab to assist with performance of appropriate ACLS treatment.
• Reviews patient record for complete documentation and compliance with Core Measures and Quality Metrics and other regulatory and quality requirements.
• Rounds on assigned inpatients to assess response to treatment, results of tests, patient/family concerns and questions and readiness for discharge.
• Communicates information to their assigned physician and documents information and/or orders as requested.
• Supports marketing of program with internal and external customers and primary care providers.
