Computer documentation proficiency to perform this task on both the outpatient and inpatient setting., Monitors patients with chronic conditions and ensures appropriate education and expectations with the next level of care to support health maintenance and reduce readmission risks., Prioritizes assigned physician workload related to inpatients, observation patients and patients presenting to the Emergency Department., Relays information between provider(s) and client(s), which may include but is not limited to communications with patients, families, pharmacies, nursing homes, consultancy office staff, insurers, and facilities. At their physician's instruction, communicates provider advice, instructions, orders and results, as applicable, including accurate documentation., Responds to all department-specific emergencies in the Heart Center and Cath Lab to assist with performance of appropriate ACLS treatment., Reviews patient record for complete documentation and compliance with Core Measures and Quality Metrics and other regulatory and quality requirements., Rounds on assigned inpatients to assess response to treatment, results of tests, patient/family concerns and questions and readiness for discharge.
