UPMC Family Hospice and Palliative Care is hiring RN for the Fayette Green and Washington County area.

Family Hospice of UPMC is seeking caring, skilled, and compassionate registered nurses (RNs) to join our team.

As a nurse for the dedicated team, you will work Monday - Friday daylight shifts from 8:00 am to 4:00 pm for Pittsburgh and surrounding areas. There will be rotations for on-call, holiday, and weekend coverage.

Final title and pay will be determined by education, experience, and certifications.

Responsibilities:

• Coordinate case management activities within an interdisciplinary healthcare team to ensure appropriate delivery of services to patients. Attend compliance training and adhere to organizational standards, policies, and procedures. Focus on customer service and strive for optimal patient satisfaction. Complete initial and comprehensive patient assessments. Receive feedback constructively and take action to improve knowledge, skills, and performance based on feedback or self-identified needs.
• Initiate, coordinate, and ensure ongoing communication with physicians and other team members, including evaluating patients and providing family-centered health teaching. Provide direct patient care, evaluate outcomes, and adjust nursing care as needed. Establish the care plan collaboratively with the physician, patient, and team members. Assign tasks to interdisciplinary team members as appropriate. Supervise and educate other nursing staff, including LPNs and Home Health Aides.
• Participate in case conferences, team meetings, and other work-related gatherings. Recognize limitations and seek assistance as needed. Complete all documentation accurately and promptly, including visit assessments and treatment plans, within the electronic system, in accordance with standards. Identify and suggest solutions to organizational issues.
• Exercise independent judgment regarding patient care needs and communicate with physicians. Participate in process improvement initiatives to enhance service quality. Contribute to weekend and on-call coverage rotations as per guidelines. Demonstrate proficiency in all aspects of patient care, including complex and specialized needs.
