Qualifications

• Graduate of a school of professional nursing.
• BLS certification required prior to employment.

Responsibilities

• Communicate with Veterans regarding health care options including local VA Medical Center and community-based eligibility.
• Serve as liaison between referring providers and specialty care services.
• Outpatient specialty clinic care coordination and patient flow management; manage appointments and referrals.
• Collaborate with clinicians and leaders across medical and surgical specialties, primary care, and Community Care.
