Job summary

North West Home Care, Inc. seeks a Program Manager & Quality Assurance Coordinator to uphold clinical and operational standards for in-home care. You will support a remote caregiver workforce and ensure client case management aligns with physician orders and state guidelines.

The role blends administrative oversight with active care management, including care planning, documentation, and occasional nursing services. Training and teamwork are emphasized in a fast-paced environment.

Qualifications

• Graduate of an accredited school of nursing.
• Current Montana or Compact nursing license, free of disciplinary action.
• Valid MT driver’s license with a clean driving record.
• One or more years of supervisory experience preferred.
• Strong proficiency with mobile applications, MS Office, email, and database/electronic care log systems.
• Working knowledge of State CFC/PAS program guidelines, Medicaid Waiver, VA, Private Pay, and SNF authorizations (must be established within 3 months).
• Strong information-gathering, analytical, and problem-solving abilities; effective communication, including active listening and feedback.

Responsibilities

• Respond to client referrals, evaluate care requirements, perform environmental assessments, and manage client intakes.
• Formulate, implement, and evaluate customized care strategies to promote or restore client health.
• Provide direct nursing services on occasion within physician orders and guidelines.
• Conduct regular follow-up visits and oversee care reauthorizations/recertifications.
• Coordinate client scheduling preferences with staffing capabilities.
• Inform clients of policy shifts and authorized care hours.
• Mentor, orient, and support PCAs, CNAs, and remote caregivers.
• Recruit, onboard, and train new caregivers; align hiring with client counts.
• Manage high-volume inquiries and investigate safety issues with HR support.
