Job summary

Meritus Health is building a high-autonomy house-calls program for patients needing wound care and longitudinal primary care in homes and at our Home Health clinic. You will own your patient panel, practice at the top of license, and serve as the clinical backbone of the model.

Qualifications

• Must hold Maryland NP license with prescriptive authority.
• 2–3 years in primary care/home-based/wound care or similar high-acuity outpatient setting.
• Ability to work independently in the field while collaborating with an interdisciplinary team.
• Valid driver’s license and reliable transportation for house calls within the service area.

Responsibilities

• Provide in-home comprehensive primary care: history, physical exam, diagnosis, and ongoing management of chronic and acute conditions.
• Deliver wound care in home and clinic, including assessment, debridement (within scope), dressing selection, and advanced therapies.
• Develop and own individualized care plans for your patient panel, aiming to prevent avoidable ED visits and hospitalizations.
• Perform periodic clinic sessions at the Home Health location for follow-up, new consults, and procedures best done in a clinic setting.
• Coordinate with Home Health nurses, PT/OT, and referring physicians for seamless transitions and aligned treatment plans.
• Order and interpret labs and imaging; adjust treatment based on results and patient goals.
• Prescribe medications, wound therapies, and durable medical equipment within scope and state regulations.
• Educate patients and caregivers on wound care, chronic disease management, medication adherence, and preventive health.
• Document promptly in the EMR with clear care plans and team communication.
• Participate in case reviews/huddles with Home Health and medical teams to refine protocols.

We’re looking for a Nurse Practitioner who wants to own their panel, practice at the top of license, and be the clinical backbone of this model.

Role Summary

In this role, you will provide in‑home visits for wound care and primary care, with the potential for some clinic‑based sessions at our Home Health department location. You will manage a defined panel of patients, coordinate with Home Health nurses and physicians, and have significant day‑to‑day independence in how you structure your schedule and care plans.

What We’re Looking For

• Licensed Nurse Practitioner (Family or Adult‑Gerontology) in Maryland, with prescriptive authority (or eligibility).
• Minimum 2–3 years of experience in primary care, home‑based care, wound care, or similar high‑acuity outpatient setting.
• Comfort working independently in the field while collaborating tightly with an interdisciplinary team.
• Valid driver’s license and reliable transportation for house calls within our service area.

How This Role Is Structured

• You will own a panel of patients attributed to you, with defined visit frequency based on clinical need and program protocols.
• Mix of home visits and clinic time; precise ratio can be tailored based on patient demand and your strengths.
• Embedded support from Home Health nurses, scheduling staff, and collaborating physicians within Meritus Medical Group.
