Job summary

Meritus Health is seeking a Nurse Practitioner to own a patient panel for in-home wound care and longitudinal primary care as part of a home health program. The role includes occasional clinic sessions, independent scheduling, and close collaboration with Home Health staff and physicians.

You will manage chronic and acute conditions, order diagnostics, and educate patients while operating within Maryland state regulations and Meritus Medical Group guidelines.

Qualifications

• Licensed Nurse Practitioner in Maryland with prescriptive authority or eligibility.
• 2–3 years in primary/home-based wound care or high-acuity outpatient settings.
• Ability to work independently while collaborating with a care team.

Responsibilities

• Provide in-home primary care and wound care with ongoing management.
• Develop and own care plans for patient panels; prevent ED visits.
• Coordinate with Home Health nurses, PT/OT, and physicians.
• Order and interpret labs/imaging; adjust treatments as needed.
• Educate patients and caregivers on wound care and chronic disease management.
• Document care plans clearly in the EMR; participate in team huddles.
