Job summary

Gilder Search Group is seeking a Nurse Practitioner for a Skilled Nursing Facility in Aberdeen, NC. This is a full-time, in-person role working Monday through Friday, 9 to 5, with no nights or weekends, overseeing a patient panel in a collaborative environment.

Qualifications

• Active North Carolina NP license in good standing.
• Experience in skilled nursing, long-term care, or geriatric primary care preferred.
• Comfortable rounding independently and managing a full patient panel.
• Strong assessment, documentation, and EMR skills.

Responsibilities

• Serve as the primary care and rounding provider for patients in the facility
• Conduct assessments, manage chronic conditions, and adjust care plans
• Document visits accurately and maintain compliance with facility and regulatory standards
• Participate in patient care meetings and coordinate with the interdisciplinary team
