You will oversee admissions, discharges, monthly stability checks, and evaluations for changes in condition, collaborating with the facility care team to maintain high-quality resident care.

This role requires an Active Texas APN licensure and DEA, a Master’s in Nursing, and strong communication and teamwork skills.

Qualifications

• Active Texas Advanced Practice Licensure (APN) and DEA.
• Master’s degree in Nursing.

Responsibilities

• Oversee admissions and discharges for SNF residents.
• Conduct monthly stability checks and evaluations for changes in condition.
• Develop advanced care plans and perform comprehensive assessments.
• Prescribe treatments as clinical lead in SNF.
• Collaborate with care team to coordinate resident care.
