Job summary

Avir Health Group is seeking a Nurse Practitioner (Skilled Nursing) to lead clinical care in a SNF setting. You will work with the facility's care team to oversee admissions, discharges, monthly stability checks, and condition-change evaluations.

You will also handle advanced care planning, comprehensive assessments, and prescribing treatments. Required are Texas APN licensure and a DEA license, plus prior nursing or clinical experience and excellent communications and teamwork abilities.

Qualifications

• Texas APN license and DEA license are required.
• Clinical nursing experience is preferred.
• Strong communication and teamwork skills are essential.

Responsibilities

• Oversee admissions and discharges for SNF residents.
• Perform monthly stability checks and evaluations of changes in condition.
