Job summary

Rite Med Group is hiring a full-time Nurse Practitioner (NP) with nursing home experience to care for the residents of AristaCare at Manchester, a skilled nursing facility in Ocean County, New Jersey.

The NP position is the on-site clinical resource for the building, collaborating with physicians, the Director of Nursing, and the nursing staff to raise quality of care and reduce hospitalizations.

Salary is $160,000 per year with full benefits; NJ NP licensure and SNF/LTC experience are required.

Qualifications

Nursing home experience required.
Active New Jersey NP license required.
Experience in SNF/LTC settings preferred.

Responsibilities

Assess, diagnose, and treat nursing home residents.
Evaluate new admissions and respond to changes in condition.
Order and interpret labs and diagnostics; manage medications.
Collaborate with attending physicians, the DON, and nursing staff.
Help reduce avoidable hospital readmissions.
