Job summary

Provider Partners Health Plan is seeking an Institutional Special Needs Plan Nurse Practitioner to provide coordinated and comprehensive care for PPHP members in designated skilled nursing facilities. You will work with the member's primary care physician to deliver high-quality care and serve as the medical liaison to the interdisciplinary team.

Qualifications

• Active NP license in state of practice.
• DEA and CDS licenses as required.
• BLS certification.
• Experience with Medicare/Medicaid.
• Minimum of 3 years clinical care experience.
• SNF/Long Term Care experience preferred.
• Strong communication and teaching skills.
• Independent thinking and decision making.

Responsibilities

• Demonstrates autonomy with comprehensive health assessments.
• Collaborates with PCP to create prevention-focused care plans.
• Adheres to PPHP documentation standards and care model.
• Observes state practice scope where licensed.
• Communicates with physicians and families during transitions of care.
• Facilitates a coordinated plan of care to avoid duplication of testing.
