Job summary

P3 Health Partners is seeking a Nurse Practitioner to support senior wellness with a geriatrics focus. In a full-time capacity, you will work with an interdisciplinary team to deliver comprehensive, value-based care to complex patients in the Salem or Roseburg area, coordinating with primary care physicians and specialized staff to optimize outcomes.

Qualifications

Active Oregon NP license and current DEA license.
Minimum 1 year NP experience in geriatrics.
Experience in home care or house calls preferred.
Ability to perform outpatient procedures such as wound care, skin tears/lacerations management, IV hydration, phlebotomy, catheterization, and epistaxis management.
Flexible, collaborative, and innovative mindset.

Responsibilities

Conduct comprehensive health assessments for P3 members in various settings.
Perform transition of care visits to ensure understanding of discharge instructions and reduce readmissions.
Perform comprehensive geriatric assessments.
Identify care gaps and communicate interventions to primary care teams.
Triage and manage non-life or limb-threatening conditions.
Collaborate with interdisciplinary teams using data-driven tools to support value-based care.
Identify key risk drivers for hospitalization.
Assist with advanced care directives and transitions to hospice.
Collaborate with Regional Clinical Medical Director as needed.
Educate P3 team members and patients on health maintenance and chronic disease management.
Maintain patient privacy per HIPAA and PHI standards.
Participate in quality improvement and documentation audits.
Maintain required trainings and chart closures.
