Job summary

Together Health in Seattle seeks a founding Advanced Practice Provider to deliver holistic primary care through telehealth, in-person, and in-home visits in and around Seattle, WA. • Ability to coordinate care with specialists and community resources.

Responsibilities

• Provide comprehensive primary care to adults via telehealth, in-office, and in-home visits.
