Job summary

Fairview is seeking a Family Medicine Physician to join the primary care team at M Health Fairview Clinic - Midway in Saint Paul, Minnesota. The position features 0.8 to 1.0 FTE, a 34 patient contact hours per week schedule, and a collaborative, team-based model within Fairview's ACO.

You will practice in a fully integrated EPIC environment with on-site specialties and experienced support staff, focusing on quality outcomes and population health management.

Qualifications

• Graduate of an accredited ACGME/AOA Family Medicine residency program.
• Board Certified or Board Prepared in Family Medicine.
• Licensed or ability to obtain Minnesota medical licensure.
• Ability to obtain DEA certification for Minnesota.
• Meet credentialing/privileges criteria.

Fairview is seeking a Family Medicine Physician to join our primary care team at M Health Fairview Clinic - Midway. This is an excellent opportunity to join a thriving practice with multiple on-site specialties and a long-tenured, highly experienced support staff.

Practice Details:

• 12 provider primary care practice with more than 7 specialists onsite.
• Patient encounters average 18-22 patients per day.
• 34 patient contact hours per week.
• As part of Fairview's Accountable Care Organization (ACO), providers practice in a collaborative, team-based model focused on quality outcomes, care coordination, and population health management.
• Call Frequency: 1:18 low intensity - phone consultation only with first tier nurse triage.
• Openings: 0.8 to 1.0 FTE (Full Time) - Flexible schedule.
• Fully integrated Electronic Medical Record (EPIC).
