NPHire is seeking a compassionate Nurse Practitioner for a locum tenens homecare assignment in Washington, focused on in-home wellness visits and risk adjustment assessments. This PRN, on-site role serves King and Snohomish counties with travel across the region.

The clinician will conduct annual wellness visits, physical exams, and medication reconciliation, while documenting findings to support quality care initiatives and risk adjustment coding.

Qualifications

  • 3 years of patient care experience preferred.
  • Active, unrestricted Nurse Practitioner license in the state of Washington.
  • Experience in primary care, adult care, geriatric care, home-based care, or risk assessment preferred.
  • Previous in-home risk assessment experience preferred.
  • Ability to work independently and efficiently in the field.
  • Comfort with EMR documentation and technology-based workflows.
  • Reliable transportation for travel to patient homes within the assigned region.
  • Strong communication skills and a patient-centered approach.
  • Bilingual skills are a plus.

Responsibilities

  • Conduct annual in-home wellness visits and comprehensive health assessments for at-risk members.
  • Perform physical examinations, vital signs collection, and medication reconciliation.
  • Evaluate functional status, pain, cognition, mental health, fall risk, and home safety.
  • Document findings accurately to support risk adjustment coding and value-based care initiatives.
  • Provide patient education related to medications, chronic conditions, and wellness goals.
  • Help identify and close quality care gaps, including recommended screenings and labs.
  • Build trust with patients and families through empathy, professionalism, and clear communication.
  • Travel throughout the assigned county as needed to meet patient care needs.
  • Collaborate with the broader care team to support high-quality outcomes in the home setting.

Locum Tenens Nurse Practitioner - Homecare and In-Home Wellness Visits in Washington. Compassionate Nurse Practitioner needed for a locum tenens homecare opportunity in Washington, supporting in-home wellness visits and risk adjustment assessments. This PRN, on-site role offers the opportunity to provide meaningful patient care across coverage areas including King County and Snohomish County. Job Details
  • Compensation: $110 per assessment.
