Job summary

Strive Health is seeking a Clinical Lead Nurse Practitioner to advance care for patients with CHF, CKD/ESKD and other chronic conditions. You will deliver longitudinal, home- and telehealth-based care, supported by advanced analytics and interdisciplinary collaboration.

You will own a dedicated patient panel, reduce hospitalizations, and drive patient engagement while navigating administrative tasks. The role offers hybrid-remote work, a competitive salary, and a 10% annual bonus with

Qualifications

Master's degree in Nursing or equivalent.
2+ years as an NP with cardiology (CHF) or nephrology (CKD) specialization.
2+ years experience with Electronic Health Records (EHR).
Current state licensure.
NB Certification.
DEA license or eligible within 90 days of hire.
BLS/CPR Certification.

Responsibilities

Manage a defined patient panel with accountability for quality, cost and outcomes, collaborating with external providers and care teams.
Provide longitudinal, home- and telehealth-based care with a patient-centered approach.
Lead care planning and ensure treatment plans align with best practices for CKD/ESKD and heart failure.
Document diagnoses for risk adjustment (HCC) and address care gaps in preventative care.
Present cases and provide clinical support for rounds and interdisciplinary meetings.
Engage with patients in multiple care settings including home, clinics and telehealth.
