Responsabilités

• Perform venipuncture follow-ups within 48 hours of discharge.
• Educate patients on vital signs monitoring before discharge.
• Coordinate wound care therapy and monitor reactions in real time.
• Log venipuncture readings into the health registry in Michigan.
• Review intake histories for potential red flags in care transitions.
• Collaborate with physicians, nurses, and healthcare staff to coordinate care.
• Assist discharge planning with case management and family involvement.
