Job summary

Your Health in Columbia, SC is seeking a qualified Nurse Practitioner specializing in psychiatry to provide telehealth and in-person care across South Carolina. The role focuses on evaluating high- and risk patients, coordinating care plans, and delivering care management to improve outcomes.

You will conduct virtual visits from home with bi-monthly facility visits in the Columbia area, and work full-time with a salary-based structure plus potential monthly bonus.

Qualifications

  • Active NP/PA/Physician license in the state of employment is required.
  • Experience delivering patient care in home health, clinic, facility, telehealth or specialty care settings.
  • Strong communication and collaboration skills within an interdisciplinary team.
  • Ability to evaluate high-risk patients and develop care plans to improve outcomes.

Responsibilities

  • Evaluate high-risk patients with complex medical needs and develop individualized care plans.
  • Coordinate care activities with the interdisciplinary team to ensure continuity of care.
  • Communicate regularly with case managers, coordinators, and other team members to coordinate patient care.
  • Participate in interdisciplinary meetings to discuss patient progress and care plans.
  • Document care management activities accurately to support patient care plans and regulatory compliance.
  • Provide patient education and resources related to conditions, treatment plans, and self-management.

Job description

We are seeking a qualified Nurse Practitioner that specializes in psychiatry to serve patients throughout the state of South Carolina. As a provider, you play a critical role in delivering exceptional patient care through telehealth and collaborate with multiple healthcare professionals to ensure comprehensive and effective patient management. Your responsibilities include evaluating high-/risk patients, coordinating care plans, and providing care management to improve patient outcomes.

Our providers conduct virtual patient visits from the comfort of their own homes with bi-monthly facility visits in the Columbia, SC area. This is a full-time salary-based position, with the opportunity to receive a monthly bonus.

  • 8-hour shift (Monday-Friday; 8:00am-5:00pm)

What Are We Looking For?

Your Health is currently looking for a Nurse Practitioner to join our growing primary care family. A successful provider will be able to perform these essential duties and responsibilities. Areas Of Responsibility
  • Evaluate high-risk patients with complex medical needs and develop individualized care plans.
  • Coordinate care activities with other members of the interdisciplinary team to ensure continuity of care and optimal patient outcomes.
  • Communicate regularly with case managers, clinical coordinators, social workers, specialists, pharmacists, health coaches, and other team members to coordinate patient care.
  • Participate in interdisciplinary team meetings to discuss patient progress, treatment plans, and care coordination efforts.
  • Foster effective communication and collaboration to facilitate seamless patient management and interdisciplinary teamwork.
  • Meet staff efficiency expectations while providing high-quality care to patients in various settings, including homes, facilities, telehealth, clinics, and specialties.
  • Document care management activities accurately and efficiently to support patient care plans and regulatory compliance.
  • Provide patients with education and resources related to their conditions, treatment plans, self-management strategies, and preventive care measures.
  • Offer support and guidance to patients and their families to enhance their understanding of healthcare options and promote active participation in their care.
  • Adhere to all relevant healthcare regulations, policies, and procedures, including HIPAA and Medicare guidelines.
  • Monitor quality measures and gaps in care, identifying opportunities for improvement and implementing corrective actions as needed.
Qualifications
  • Nurse Practitioner (NP), Physician Assistant (PA), or Physician with a valid license to practice in the state of employment required. License must be in good standing.
  • Demonstrated experience in providing patient care, preferably in a home health, clinic, facility, telehealth, or specialty care setting.
  • Strong communication, collaboration, and interpersonal skills, with the ability to work effectively within an interdisciplinary team.
  • Ability to evaluate high-risk patients, develop care plans, and coordinate care activities to meet patient needs and improve outcomes.
  • Commitment to delivering compassionate, patient-centered care and promoting a culture of excellence in healthcare delivery.
  • Strong organizational and time-management skills, with the ability to multitask and prioritize responsibilities effectively.
  • Ability to read and communicate effectively.
  • Strong written and verbal skills.
  • Basic computer knowledge.
  • Ability to manage and demonstrate effective leadership skills.
  • Should demonstrate good interpersonal and communication skills under all conditions and circumstances.
  • Ability to foster a cooperative work environment.
  • Team player with ability to manage multiple responsibilities and demonstrate sound judgment.
  • Must be able to work flexible hours and travel between offices, facilities, etc. Must be a licensed driver with an automobile that is insured in accordance with state and/or organizational requirements and is in good working order.
