Role Overview

Your Health is looking for a Nurse Practitioner to join our primary care family. The successful Provider will perform essential duties in a variety of settings, including homes, facilities, telehealth, and clinics.

Areas of Responsibility

• Evaluate high‑risk patients with complex medical needs and develop individualized care plans.
• Coordinate care activities with interdisciplinary team members to ensure continuity of care.
• Communicate regularly with case managers, coordinators, social workers, specialists, pharmacists, and health coaches.
• Participate in team meetings to discuss patient progress and care coordination efforts.
• Facilitate effective communication and collaboration to support seamless patient management.
• Maintain staff efficiency while delivering high‑quality care across various settings.
• Accurately document care management activities to support patient plans and regulatory compliance.
• Provide patient education and resources related to conditions, treatment plans, self‑management, and preventive care.
• Offer support and guidance to patients and families to enhance understanding of healthcare options.
• Adhere to all relevant healthcare regulations, policies, and procedures, including HIPAA and Medicare guidelines.
• Monitor quality measures, identify gaps, and implement corrective actions as needed.

Qualifications

• Valid Nurse Practitioner (NP), Physician Assistant (PA), or Physician license in good standing in the state of employment.
• Demonstrated experience in patient care, preferably in home health, clinic, facility, telehealth, or specialty setting.
• Strong communication, collaboration, and interpersonal skills within interdisciplinary teams.
• Ability to evaluate high‑risk patients, develop care plans, and coordinate care activities to meet patient needs.
• Commitment to delivering compassionate, patient‑centered care and a culture of excellence.
• Strong organizational and time‑management skills, with the ability to multitask and prioritize effectively.
• Basic computer literacy.
• Leadership skills and ability to foster a cooperative work environment.
• Flexibility to work varied hours, travel between sites, and a licensed driver with an insured, reliable vehicle.
