Job summary

The Care Team Hospice is seeking a compassionate Nurse Practitioner to provide expert clinical assessments, symptom management, and end-of-life care for hospice patients in their homes and facilities. You will complete timely Face-to-Face visits for hospice recertifications and collaborate with an interdisciplinary team to optimize care.

Requirements include Indiana NP license, 2+ years of AP/clinical experience (hospice preferred), CPR, TB testing, and a valid driver’s license.

Qualifications

  • Current Nurse Practitioner license in Indiana.
  • Graduate of an accredited Nurse Practitioner program.
  • Minimum of two years of advanced practice or clinical nursing experience; hospice/palliative care preferred.
  • Strong clinical assessment, communication, and critical thinking skills.
  • Ability to work independently while collaborating with an interdisciplinary team.
  • Knowledge of hospice philosophy and Medicare hospice regulations preferred.
  • Valid driver's license and reliable transportation.
  • Ability to pass a criminal background check and MVR screening.
  • Current CPR certification.
  • Current TB testing per agency policy.

Responsibilities

  • Complete timely Face-to-Face (F2F) encounters for hospice recertification per Medicare guidelines.
  • Perform comprehensive clinical assessments and communicate findings to the interdisciplinary team.
  • Collaborate with physicians, nurses, and care team to develop and support plans of care.
  • Educate patients, families, and caregivers on disease progression, symptom management, and end-of-life care.
  • Maintain accurate documentation and ensure compliance with hospice regulations and policies.
